Rental Applicant Screening Disclosure & Authorization

This form must be reviewed, signed, and dated by the Applicant BEFORE any driver's license or driving record check is performed. It is intentionally kept separate from the Long-Term Vehicle Rental Agreement so that the Applicant's consent to screening is clear, standalone, and not buried inside other contract terms.


1. PURPOSE 

SCICA Fleet Operations (“Company”) uses driver's license verification, driving record (MVR) checks, and verification of active rideshare/delivery platform standing (including driver rating) to evaluate rental applications and protect both the Company's vehicles and its renters. This form discloses what will be checked and obtains your written authorization before any check is performed, consistent with the federal Fair Credit Reporting Act (FCRA) and applicable New York law.


2. WHAT MAY BE CHECKED 

  • Driver's license validity, class, and status, verified against state DMV records. 

  • Driving history / Motor Vehicle Record (MVR), including violations, suspensions, and accidents on file with the state. 

  • Verification of your active rideshare/delivery platform account(s) and driver rating (e.g., Uber, Lyft, DoorDash, Instacart), to confirm good standing on the platform(s) you intend to use this vehicle for.

  • Depending on the specific circumstances of your application, the Company may contact you directly to request additional information needed to complete the screening process. 


3. YOUR RIGHTS UNDER THE FCRA 

  • You have the right to know whether a consumer report was used in any decision made about your application. 

  • If the Company denies your application, or offers different terms (such as a higher security deposit), based in whole or in part on information in a consumer report (such as your driving record), you will receive an Adverse Action Notice identifying the consumer reporting agency used, stating that the agency did not make the rental decision and cannot explain the specific reason for it, and explaining your right to a free copy of the report and your right to dispute inaccurate information directly with that agency.

  • You have the right to obtain a free copy of your credit report from the reporting agency within 60 days of receiving an Adverse Action Notice. 

  • You have the right to dispute incomplete or inaccurate information directly with the consumer reporting agency. 

4. AUTHORIZATION 

By signing below, I authorize SCICA Fleet Operations to obtain my driver's license status and driving record (MVR) from a consumer reporting agency, and to verify my active rideshare/delivery platform standing and driver rating, for the purpose of evaluating my rental application. I understand this authorization is voluntary, but that the Company may be unable to approve my rental application without it. 

(A minimum driver rating, consistent with current platform standards, may be required for approval — the Company will advise if your rating does not meet this threshold.)
